Virus Ebola, il Congo desta preoccupazione.

La malattia da virus Ebola (EVD) è una malattia grave, spesso fatale, con un tasso di mortalità di circa il 50%. (fino a toccare punte del  90% in epidemie passate)

La malattia colpisce gli uomini e i primati (scimmie, gorilla, scimpanzé).

L’Ebola è apparsa la prima volta nel 1976 in due focolai contemporanei: in un villaggio nei pressi del fiume Ebola nella Repubblica Democratica del Congo, e in una zona remota del Sudan.

L’origine del virus non è nota, ma i pipistrelli della frutta (Pteropodidae), sulla base delle evidenze disponibili, sono considerati i probabili ospiti del virus Ebola.

L’Ebola si trasmette nella popolazione umana attraverso lo stretto contatto con sangue, secrezioni, tessuti, organi o fluidi corporei di animali infetti.

Lo scorso 8 maggio 2018 l’Organizzazione Mondiale della sanità (WHO) è stata informata dal Ministero della Sanità della Repubblica Democratica del Congo di due casi confermati dal laboratorio da virus Ebola che si verificano nella zona sanitaria di Bikoro, nella provincia di Equateur.

I casi sono stati trovati anche nei vicini Iboko e Mbandaka.

Dal 4 aprile al 17 maggio 2018, sono stati segnalati 45 casi di Ebola Virus Disease EVD, compresi tre operatori sanitari, e sono stati segnalati 25 decessi.

Di questi 45 casi, 14 sono stati confermati.

La maggior parte di questi casi è avvenuta nella remota zona di salute di Bikoro, anche se un caso confermato è a Mbandaka, una città di 1,2 milioni, che ha implicazioni per la sua diffusione.

Nove paesi confinanti, tra cui Congo-Brazzaville e Repubblica centrafricana, sono stati avvisati che sono ad alto rischio di diffusione e sono stati sostenuti con attrezzature e personale.

il Comitato ha concluso questi obiettivi:

  • L’epidemia di Ebola nella Repubblica Democratica del Congo ha diverse caratteristiche che destano particolare preoccupazione:
  • il rischio di una diffusione più rapida dato che l’Ebola si è ora diffusa in un’area urbana; che ci sono diversi focolai in aree remote e difficili da raggiungere; che il personale sanitario sia stato infettato, il che potrebbe essere un rischio per un’ulteriore amplificazione.
  • Il rischio di diffusione internazionale è particolarmente elevato in quanto la città di Mbandaka si trova in prossimità del fiume Congo, che ha un significativo traffico regionale attraverso i confini porosi.
  • Ci sono enormi sfide logistiche date le scarse infrastrutture e la posizione remota della maggior parte dei casi attualmente segnalati; questi fattori influenzano la sorveglianza, il rilevamento e la conferma dei casi, la tracciabilità dei contatti e l’accesso ai vaccini e alle terapie.

Advanced therapy medicine Imlygic indicated to treat certain stages of melanoma

The European Medicines Agency (EMA) has recommended authorising Imlygic (talimogene laherparepvec) for the treatment of adults with melanoma that cannot be removed by surgery and that has spread either to the surrounding area or to other areas of the body (regionally or distantly metastatic) without affecting the bones, brain, lung or other internal organs.

Imlygic is a first-in-class advanced therapy medicinal product (ATMP) derived from a virus, that has been genetically engineered to infect and kill cancer cells. The recommendation was made by the Committee for Medicinal Products for Human Use(CHMP) based on an assessment carried out by the Committee for Advanced Therapies(CAT), the Agency’s expert committee for ATMPs.

Melanoma is the most aggressive form of skin cancer and the leading cause of death from skin disease. The main risk factor for developing melanoma is ultraviolet (UV) light and unprotected sun exposure. In 2012 more than 100,000 Europeans were diagnosed with melanoma and it is estimated that around 22,200 have died from the disease. If melanoma is localised and detected early, it can often be removed by surgery and patients have a very good chance of survival. However, patients with melanoma that has spread have a poor prognosis. For decades, standard chemotherapy was the only available treatment for these patients. In the last three years, the authorisation of targeted treatments, including immunotherapies, BRAF V600 inhibitors and MEK inhibitors, have changed the therapeutic landscape and increasingly benefited patients. However, there is still an important need for new treatments with acceptable safety profiles to continue to improve the outlook for patients with this serious life-threatening disease.

Imlygic is derived from a virus (herpex simplex virus-1) that has been genetically engineered to infect and replicate (make copies of itself) within cancer cells and to produce the protein GM-CSF. It is thought to work in two ways: it enters the tumour cell and uses the cell’s energy stores to replicate, eventually overwhelming the cell and causing it to die. By producing the protein GM-CSF it also stimulates the patient’s immune system to recognise and destroy tumour cells. Once an infected tumour cell dies, copies of the virus are released into the patient’s bloodstream to infect more tumour cells. Although Imlygic can enter healthy cells, it is not able to replicate in these healthy cells and so it does not kill them.

Imlygic is recommended to be injected directly into the melanoma lesions. Theefficacy of Imlygic compared with injecting GM-CSF under the skin, was assessed in one randomised controlled trial in adults with unresectable (cannot be removed by surgery) regionally or distantly metastatic melanoma. The study enrolled 436 patients and the efficacy of 295 patients treated with Imlygic was compared with 141 patients treated with GM-CSF. Analysis of the subset of patients in the study whose disease had not spread to the lung or other internal organs (249 patients with stage IIIB, IIIC and IVM1a disease) showed a benefit in patients treated with Imlygic compared with patients treated with GM-CSF: 25% of those patients treated with Imlygic responded positively (durable response) compared with 1% of those patients treated with GM-CSF. A durable response was defined as disappearance of the tumours or at least 50% reduction of tumours lasting at least six months until patients’ health declined or they required subsequent therapy. Exploratory analysis in these patients suggested improvements in survival in patients treated with Imlygic, however this is not yet fully clear. Imlygic has also not been compared with other recently approved medicines for melanoma, which have shown beneficial effects on survival.

Overall the CHMP and CAT considered that Imlygic is relatively well-tolerated in patients with cancer and concluded that the medicine’s benefits outweigh its risks. A follow-up plan to monitor the efficacy and safety of Imlygic was agreed by the CHMPand the CAT.

The company received scientific advice from the CHMP on quality, non-clinical and clinical aspects of the application. This is one of the Agency’s main tools to facilitate and stimulate research and development within the European Union (EU).

The opinion adopted by the CHMP at its October 2015 meeting is an intermediary step on Imlygic’s path to patient access. The CHMP opinion will now be sent to the European Commission for the adoption of a decision on an EU-wide marketing authorisation. Once a marketing authorisation has been granted, each Member State will take a decision on price and reimbursement based on the potential role/use of this medicine in the context of its national health system.
